Treatment of Vasopressin-Sensitive Diabetes Insipidus with Chlorpropamide

Abstract

Oral chlorpropamide therapy was assessed in 9 years-old male who entered with the complaints of polyuria, polydipsia and nocturia for 9 months.
A significant reduction in urine volume and increase in urine specific gravity occurred by its use. Mean 24 hour urine output during control periods was 5,460 ml(range, 5,300~5,700 ml) and mean urine specific gravity was 1,005 (range, 1,003~1,007) on the time of admission. On chlorpropamide therapy, mean output fell to 1,810 ml(range, 1,200~2,500ml) and mean specific gravity rose to 1,013 (range 1,011~1,014).
For many children with vascpressin-sensitive diabetes insipidus, chlorpropamide seems to be an effective form of therapy. The most recent experimental evidence suggests that the antidiuretic action of the drug may be due to an effect on the renal tubule, where by the latter¢¥s sensitivity to small amounts of vasopressin may be enhanced.
